Where do you store/keep sensitive customer information?

Files are kept in locked cabinets in my office. I use paper files.There is a security alarm system in the building. I share the office with other therapists and psychiatrist.

How do you use the information you store about your customers?

To assist me in my work and provide information to and for my clients. Information is strictly confidential and is only released with a signed Authorization for Release of information, including the length of time the release remains in place. I prefer to provide a summary of information of my assessments if a client requests information, to provide relevant information that the client can comprehend. Clients are provided with information under federal HiPPA that describe situations when personal information can be released, such as in incidents of child abuse or information necessary to protect the life and safety of clients and others. I keep Progress Notes, which are document each session, and Process Notes, which are notes to myself about impressions or transference issues.

Do you share, sell, or otherwise provide copies of your customer information to other businesses? Under what circumstances would you do this?

I will provide relevant information to other service providers with the client's Authorization for Release of Information. I will write letters for a client to support their applications for Social Security Disability, Insurance claims, or notes documenting the need for support animals.

If a customer wanted to request a copy of all of the information you have on file for them, how should they do that?

They will need to make the request in writing and pay a fee for copies of case file notes. I would ask the client the reason for the request and prefer to provide a summary, with diagnosis and treatment provided than copies of all case notes.
